About our DVA services

With our veteran health service, we support all DVA, Veterans, Australian Defence Force members and their families, we understand the challenges that they may face. Our Medical Practitioners, Allied Health and Staff will support your journey to good health and vitality.

Rosslea Medical Centre is the number one practice in Townsville, QLD and Australia Wide who provide the highest patient numbers for our veterans. We are extremely proud to support this service.

CVC PROGRAM

  • Assisting Veterans with booking appointments through the patient coordinator on their personal work mobile.
  • Monthly phone call to stay connected with health support.
  • Home visits from the patient coordinator as required by the veteran; and
  • Provides a link between the patient coordinator and your General Practitioner for any urgent script renewals or referrals to any specialist/allied health support.

CVC Program

What is the Coordinated Veteran Care (CVC) program?

The Coordinated Veteran Care (CVC) program is for veterans that have served in the Australian Defence Force and have a Department of Veteran Affairs (DVA) gold card and/or white card and have accepted chronic mental health conditions.

The CVC program was established to reduce the instances of preventable hospitalisation of veterans in the community. Thus, the program’s objectives are to connect veterans with a nurse coordinator to aid in providing continuous high-level care.

What is a Patient Coordinator?

The Patient Coordinator is a qualified registered/enrolled nurse who has a personal work mobile, and their role is to assist the veteran in adhering to their personalised health care plan.

Our CVC program team consists of practice nurses, patient coordinator, and your treating general practitioner (GP).

Gaynor Williams

My name is Gaynor Williams, and I am the CVC patient coordinator at Rosslea Medial Centre (RMC).

My passion for helping veterans in general practice comes from over 30 years of being an army wife.

I know the mental and physical demands serve life has, not only on the veteran but the family.

My role as the patient coordinator is to ensure you receive the highest level of care possible. I will act as the link between the veteran and your general practitioner to ensure all goals set out are achieved.
