Our community nursing care services
Our services are designed to promote health, prevent disease, and provide care and support for people of all ages, including those with chronic illnesses, disabilities, or other healthcare needs. Community nurses play a crucial role in delivering these services, offering a holistic approach that addresses physical, mental, and social well-being.
Our services are based on your unique needs, choices and interests. A care plan will be developed to meet your individual needs, with the aim of supporting independent living, managing health conditions, and achieving health goals within the familiar and supportive environment of the community. This may include general nursing care, specialist nursing in palliative care, chronic disease management, continence advisory and wound care.
We also provide personal care including hygiene (bathing, grooming, oral and hair care), dressing and mobility assistance.

About Our story, mission and values
Dr Francisco (Paco) Munoz opened Rosslea Medical Centre on 21 March 2011 as a family medical practice. Dr Paco is very passionate about the care of his patients, and he quickly identified great potential to improve the level of care his patients receive in the community. For many years it has been a goal to establish a branch of Community Nursing to enhance the continuity of care provided by Rosslea Medical Centre, and we are proud to announce Rosslea Medical Centre Community Care.
At Rosslea Medical Centre Community Care we aim to provide exemplary physical, emotional and spiritual care for each of our clients and their families.
Our goal is to positively influence our clients and the local community toward leading a healthier lifestyle, through facilitating improvements in health and wellbeing.
We provide a work environment where each person is valued, respected and has the opportunity for personal growth and professional development.
The commitment of our staff, practitioners, and community partners to our mission, will ensure we continue to maintain a quality of presence and tradition of caring in the community we serve.

Code of conduct
The Australian Government has created a Code of Conduct to improve the safety, health, wellbeing and quality of life of aged care consumers.

2. Act in a way that treats people with dignity and respect and values their diversity.
3. Act with respect for the privacy of people.
4. Provide care, supports and services safely and competently, with care and skill.
5. Act with integrity, honesty and transparency.
6. Promptly take steps to raise and act on concerns about matters that may impact the quality and safety of care, support and services.

all forms of violence, discrimination, exploitation, neglect and abuse sexual misconduct.

Your responsibilities
Treat others with respect – You, your family members, visitors and staff are expected to be respectful and considerate. Any kind of violence, harassment or abuse towards staff or others is not acceptable.
Make your home safe for the nursing staff caring for you – please refrain from smoking near staff, and ensure pets are kept away.
Provide relevant information about yourself so we can provide you with safe and quality care. Know that your health could change, and you should let the nursing staff know about any changes, so they know how to care for you.
